Duties & Responsibilities/Principal Activities:
- Responsible for ensuring all phases and activities of the Wraparound Process are done to high quality and fidelity – accountable to family team.
- Plan and facilitate the monthly Child and Family Team Meetings, in partnership with the family, natural supports, informal supports, and formal supports.
- In partnership with the FSS, provide face-to-face direct service to families in their home or community, as needed in implementing High Fidelity Wraparound
- Develop and coordinate Plan of Care and Crisis Plan with the family team.
- Provide outreach and engagement to families upon referral to the program by conducting initial phone contact and the intake process with the assistance of the Family Support Specialist (FSS)
- Complete required evaluations, including Child and Adolescent Needs and Strengths (CANS) assessment, Caregiver Strain assessment, and other national evaluation tools to develop a care plan
- In collaboration with the FSS, assist family in developing a list of supportive individuals they want to attend the Family Team Meetings

- Conduct weekly and as needed one on one Personal Support sessions with each assigned client.
- Assess the general need of assigned clients and conduct appropriate Groups which will support clients health and wellbeing.

Qualifications: Bachelor’s degree in early childhood education or related field including child development, special education, or developmental psychology, with a minimum of three (3) years experience in field. - Gather all information for Behavioral Health Safety Net (BHSN) and enter into EHR as well as State Portal to manage BSHN admissions.
- Provide monthly care coordination for those enrolled in BHSN Program for the purpose of linking to clinically indicated services or to benefits that would provide an alternative payer source for services. Services can be:
- Completing related documentation to identify the needs of the individual.
- Monitoring and follow-up activities for care

- Monitoring and assisting with follow-up for therapy, medication management, pharmacy assistance, lab-work and community engagement.

- Facilitate internal referrals and manage special request of outpatient service recipients by reviewing staffing availability in other TNV programs.
- Manage scheduling for all clinical services for service providers, including therapy, psychiatry, and intake services and adjust need to accommodate service recipients and service provider using Microsoft Office and electronic health record software.
- Manage the scheduling phone line and be available during business hours to take calls pertaining to scheduling and medical records.
- Follow-up with clients following clinical intake appointment to ensure satisfaction and that the intake process has been completed.
- Act as a communication liaison between outpatient service providers and Clinical Director to ensure cases are effectively coordinated and successfully completed.

- Links and oversees linkage of youth/families with local community resources.
- Coordinates and assists with satisfaction surveys.
- Coordinates and performs all necessary data recording and recordkeeping.
- Oversees and teaches effective advocacy skills, in keeping with system of care principals.
- Coordinates and provides timely reports to funding source(s) as well as individual screening sites.
- Assures all Youth Screen activities meet or exceed national standards.
- Supervises and supports program staff, interns and volunteers.
- Models and teaches effective advocacy skills, in keeping with system of care principals.
- Collaborates with the Program Director on program scope of services, implementation and sustainability.
- Ensure that we conform to CARF standards applicable to the services provided.
- Other duties and/or tasks as assigned.
