How We May Use Your Information
We may use your information to provide and coordinate your care, schedule appointments or communicate with you (with your consent), bill insurance/Medicaid or grant funders, and meet legal, licensing, and funding requirements. We only share the minimum necessary information, and many uses require your permission.
Extra Protection for Substance Use Records
Some records related to substance use treatment have additional federal protections. These records usually cannot be shared without your written consent or a court order.
Your Rights
You have the right to see and get copies of your records (paper or electronic), ask for corrections, request limits on sharing, require us not to bill your health plan for services you paid for in full, receive a paper copy of the full Notice, file a complaint, and receive breach notification if your unsecured PHI is compromised.
